I am wanting to start my 11' x 9' 6'' free standing deck, but have numerous questions.
1. Building on a existing concrete slab.
2. Attaching to concrete slab with https://www.lowes.com/pd/USP-4-in-x-4-i ... se/3365734.
3. I need to know where to set these anchors.
4. The post will be slightly different heights to compensate for a slope in the slab.

That is NOT the anchor base you want, you want an adjustable post base such as one like this:

An APB is more forgiving to locate and it has a riser plate for the post to keep it up off the concrete and out of the water, which helps prevent rot. They can be anchored with TapCon screw, expansion bolts, concrete nails, shot driven nails, your choice.

Where the posts are located will depend on the design of the deck framing, what is your plan with that?

Yup I agree these other post bases are way better.
Post placement and quality will all depend on joist and beam sizes .
If your deck joists are running the 9-6" direction then they could be 2x6 @16" O.C. and cantilevered over the beams 12"
The beams could be double 2x6 with 3 posts .
These are my suggestions based on guessing about your design

This is the basic design I am following.
It is setting at the front entrance to a mobile home so there is no attaching it to the house.
I tried to add a picture of the site, but it would not allow me to do so.
